If your insurance company is not listed, we are not contracted with them, however you may ask to speak with the billing department to clarify any questions you may have regarding out-of-network benefits.
This office verifies all insurance coverages prior to your scheduled appointment. In the event we cannot verify insurance benefits, your appointment may be rescheduled until we can obtain current information.
Patients should provide secondary insurance plan information at check-in. We will file secondary insurance as a courtesy.
